Getting started

From first call to first visit

You don't need to coordinate everything yourself. Here is how care begins once you or your doctor reaches out.

  1. Call or get referred

    You, a family member, your doctor or the hospital contacts our intake team.

  2. We check coverage

    We verify your insurance and confirm the physician order and face-to-face visit.

  3. Start-of-care visit

    A registered nurse or therapist visits your home, completes an assessment and reviews your medications.

  4. Your plan of care

    Your doctor signs a plan listing your visits, goals and which clinicians will see you.

  5. Ongoing visits

    Your team visits on schedule, updates your doctor and plans for discharge when goals are met.

Who qualifies

Is home health right for you?

  • Your doctor orders skilled nursing or therapy services.
  • Leaving home takes considerable effort, such as needing a walker, wheelchair or another person's help.
  • You recently left a hospital or rehab facility, or a chronic condition has gotten worse.
  • You've had a fall, a new diagnosis or a change in medications that needs monitoring.

When you qualify, Medicare covers home health services at no cost to you. We also work with Ohio Medicaid and many managed care plans.
